EMPLOYER REQUIRED SUBSTANCE ABUSE ASSESSMENT
Employer required drug & alcohol assessments apply to situations when the employee has violated company alcohol and/or drug policy in the workplace due to reasonable suspicion, an accident on the job, and in situations where the employer suspects an employee has a substance abuse related problem outside the workplace and might need help.
Our clinical assessments evaluate the employee’s readiness to return to work and make specific clinical recommendations for treatment (if applicable). If treatment is recommended, we provide a time frame within which the employee and employer can expect the employee to return to work, assuming employee’s compliance.
All assessment determinations are made using American Psychiatric Association DSM-5 diagnostic criteria.
Your assessment report is provided to you for your own purposes and it is at your discretion for whom you allow to review it.

Weekday standard service – $320.00
(Completed 4-5 business days after interview)

Note: * Our support staff can help you set up a simple webcam app to use on your computer or tablet** Some assessments may require that your evaluator make brief telephone contact with a witness on your behalf
Copy of any employer instructions or paperwork needing completed for the employer
Copy of your positive drug test if applicable (if you are unsure speak to your evaluator)
Note: You will need to supply (as applicable) the preceding documents before your assessment can be finalized. You may register and complete your interview before submitting these documents but we must receive these documents in order to complete your assessment.
